We are currently taking on new patients. To register with the surgery you must complete and sign a registration form. If you are currently registered at a surgery in Maltby, we strongly advise staying with them as it is in your best interests and not appropriate to be changing GPs at this time.

You will need to complete and sign the registration form as a minimum and drop it off at Reception. If you require access to online services you will need to provide photo ID. Please phone and speak to Reception regarding this.
Once the surgery reopens you will need to attend the surgery for a new patient screening appointment with our Health Care Assistant.
To register with Blyth Road Medical Centre, please call in at Reception with ID, including proof of current address and ask for a New Patient Registration Form.
When you have completed a registration form please contact the surgery to make an appointment with the Healthcare Assistant for a New Patient Screening in order for us to finalise your registration. You will not be able to see a doctor until this form is completed and you have seen the Health Care Assistant.
